Abstract

Lewolaga Health Center is an outpatient health center located in Titehena District. This health center provides health services to the community around the area. Patient data collection at the Lewolaga Health Center still uses the manual method so that it becomes an obstacle for the counters in collecting large numbers of patient data.. Based on the existing problems, it is necessary to build a website-based patient data application to assist the counter section in terms of patient data collection. The method used for the development of this system using the Waterfall method. The application is built using the PHP programming language and MySQL Database.The results of this study are an application that helps the counter in terms of patient data collection, paramedics in handling patients and the head of the puskesmas to view patient data every month.
