Abstract

Patient data collection at the Sagu health center has not yet used online- based computerized patient data collection, even for the process of recording drugs and recording examination results, it still uses manual or written methods. This can cause problems such as the loss of patient data and take a long time in the process of retrieving patient data, so it can interfere with the patient service process. Of all the existing problems, it is necessary to have an effective and efficient Web-Based Sagu Health Center Patient Data Collection Application in helping to speed up and simplify the patient data collection process. The model used in software development is a waterfall model with systematic approaches to system development. The results obtained through this web-based patient data collection application can collect patient data, process officer data, process drug data and print examination results, so that health services at the Sagu health center are of high quality. In addition, this information system is the right choice of advice in delivering information, especially for patient data collection at the Sagu Health Center so that the officers are able to work quickly and appropriately in serving patients and the community.
